EARLY INTERVENTION

Fairhaven's Early Intervention Program provides appropriate and comprehensive family-centered services for infants and toddlers (birth - 3 years) who demonstrate one or more developmental delays. There is full-day programming available at the Fairhaven School. Home-based developmental service, service coordination, and home visits are also provided.

The purpose of the program is to meet the identified needs of infants and toddlers and their families with a goal to promoting the child's development. The Early Intervention Program is coordinated with families and other community agencies so that services are flexible and responsive to family wishes.

Who Is Eligible:
County Board shall provide services and supports to children under three years of age with developmental delays or disabilities and their families. To be eligible for County Board services, a child would demonstrate a developmental delay in one or more of the following areas, as measured by a research-based developmental evaluation tool and clinical opinion:

  • cognitive development
  • physical development, including vision, hearing and nutrition
  • communication development
  • social or emotional development
  • adaptive development (self-help); or

Having a diagnosed physical or mental condition. 

Child Evaluation and Assessment:
Licensed, certified Fairhaven personnel will conduct a comprehensive evaluation/assessment, in a play-based setting involving parents and professionals, to determine eligibility. There is no cost to the family.

Services Offered:
Program services may include:

  • Speech-Language Therapy
  • Physical Therapy
  • Occupational Therapy
  • Play-Based Peer Interaction
  • Home-Based Services
    (up to 18 months)
  • Center-Based Services
  • Cognitive Development
  • Social-Emotional Development
  • Sensory Development
  • Nursing
  • Transportation

Children and families may participate in Fairhaven's Early Intervention Program on a full day or 2 days per week basis. The frequency and location of services are determined by the Individual Family Service Plan (IFSP) Team and are based on the child's age and developmental needs, emotional needs, physical stamina, and, above all, the wishes of the family.

In order to receive services from the Trumbull County Board of Developmental Disabilities, a person must:

  • be a resident of Trumbull County;
  • have documented disability, a physical or mental impairment other than mental illness, and
    that disability:
  • must have manifested prior to the age of 22 and
  • is likely to continue indefinitely;
  • have completed an assessment called the Ohio Eligibility Determination Instrument (C/OEDI), and documented substantial functional limitations in several areas of the instrument.

 

The board, leadership, and employees of the Trumbull County Board of Developmental Disabilities are steadfast in maintaining and creating programs and services that meet the unique needs of individuals and their families.
