Seeking HPC staff to service an active 21 year old male in the Niles area. Individual enjoys walks, bike rides, store trips, zoo trips, volunteering, swimming and watching movies. Individual currently receives 54 hours per a week of HPC IO waiver services and is looking for additional staff to cover 10-15 hours per a week. Individual has PICA and requires constant visual supervision in the home and community. His family is open to working with both agencies and independent providers.

His weekly hours are as follows:

Monday - 8:00 a.m. to 6:00 p.m.

Tuesday - 8:00 a.m. to 6:00 p.m.

Wednesday - 8:00 a.m. - 6:00 p.m.

Thursday - *

Friday - 8:00 a.m. - 6:00 p.m.

Saturday - *

Sunday - *

* Additional 10 hours per a week to use on Thursday, Saturday, or Sunday.

Seeking HPC staff to work with active 48 year old female who lives in the Warren-Howland area. She is routine oriented. At home she likes to keep busy, having company, housekeeping duties, listening to music, watching favorite shows and girly things. Also she enjoys going places in the community such as shopping/walking the mall, swimming, out to eat, going to musical shows. She has Cerebral Palsy and a visual impairment; ambulates on her own with assistance in community for vision. She is not left alone; visual supervision when in community, listening when in home. She requires assistance with her personal care needs. She is difficult to understand.

Needed days/times are:

Every Wednesday - 2:00 p.m. to 9:00 p.m.

2nd Weekend of the Month:

Friday - 10:00 p.m. to 11:59 p.m.

Saturday - 12:00 a.m. to 10:00 a.m.

Saturday - 10:00 p.m. to 11:59 p.m.

Sunday - 12:00 a.m. to 10:00 a.m.

Looking for a HPC provider in the Mineral Ridge Area with medical add-on for an adult male who is on the I.O. Wavier. He needs g-tube feeding, colostomy care, bathing, dressing, checking respiratory status and vitals, total personal care assistance and ambulatory with assistance. He lives in a one-floor apartment with a walk-in bathtub.

Typical HPC schedule:

Monday - 8:30 a.m. to 8:30 p.m.

Tuesday - 7:30 a.m. to 3:30 p.m.

Wednesday - 8:30 a.m. to 8:30 p.m.

Thursday - 7:30 a.m. to 3:30 p.m.

Friday - 8:30 a.m. to 2:30 p.m.

Saturday - 8 a.m. to 5 p.m.
